Insurance
Eligibility for the insurance described in this section varies by staff group and appointment. Please refer to your Benefit Summary and Enrolment Guide for details and links to application forms. The table below describes who receives the benefit and when you can designate a beneficiary.

Your designated beneficiary(ies). If there is no designation on file, the benefit is paid to your estate. You can designate or change your beneficiaries at any time. Please submit a signed Designation of Beneficiary form to Shared Services via the Staff Service Centre. |
| Basic Accidental Death and Dismemberment (AD&D) |
For loss of life, the benefit is payable as per your basic employee life insurance designation. Loss or injury benefits are payable to you. |
| Optional AD&D |
Your designated beneficiary(ies). If there is no designation on file, the benefit is paid to your estate. You can designate or change your beneficiaries at any time. Please submit a signed Optional AD&D form to Shared Services via the Staff Service Centre. |

Pension
If you have a spouse, they are your pension beneficiary by law. You may designate whomever you wish as contingent beneficiaries.Your pension plan provides more details on the definition of a spouse and how to update your beneficiaries.
Public Service Pension Plan
Your Pension Profile (YPP) is a secure, online portal for all Public Service Pension Plan (PSPP) members. The portal is the only way you can designate or update your beneficiary or beneficiaries paper forms are no longer accepted. To use YPP, first register for your account. For any questions, call PSPP Member and Pension Inquiries at 1-877-453-1777.
University Academic Pension Plan
If you are an active member, your designated beneficiary information is maintained by Shared Services. The University Academic Pension Plan (UAPP) Designation of Spouse and Non-Spouse and Beneficiary Change forms are available at UAPP . Submit completed forms through the Staff Service Centre.
