Student Placement Agreement
One accreditation standard for all Faculties of Pharmacy across Canada (as set forth by the Canadian Council for Accreditation of Pharmacy Programs) states that an agreement must be in place to formalize the relationship between pharmacy students and the practice site. A standard agreement has been created with input from legal departments at the University of Alberta. This agreement outlines the rights, privileges, obligations, and responsibilities of the Faculty and the experiential practice site. The consultation received has resulted in a concise and practical agreement that outlines the following elements:
- Pharmacy Practicum Program including determination of placement duration and number of students accepted at a practice site
- Supervision and evaluation responsibilities
- Cancellation terms
- Worker's Compensation for students provided by the University
- Insurance required by the practice site
Please review the Single-Faculty Placement Agreement. The Faculty requires an original signed copy of this placement agreement from both the placement site (either corporately or the individual site) and the Dean of the Faculty of Pharmacy and Pharmaceutical Sciences.
