Latest Updates to HR Forms

As part of the ongoing Continuous Administrative Service Improvement Program (CASIP) initiative to review and improve HR forms and checklists, Shared Services has made changes to the forms listed below. All forms have been updated to have a consistent layout that aligns with form standards and U of A branding in addition to the specific form updates noted in the  HR Forms & Checklists Improvement Log

  • Pay Action Form - Graduate Student 
  • Time Approvers Checklist 
  • Support Staff Employee Probationary Review
  • Support Staff Performance Appraisal
  • Request for Out of Country Payment
